DECLARATION OF CONSENT TO THE PROCESSING OF PERSONAL DATA AND THE USE OF IMAGE

I, the undersigned, consent to the processing of my personal data in the field of name and surname and data of the employer or the represented institution for the purposes of conducting online and offline conference "Debating the future of Europe: contributing to a roadmap for a more united Union" by the Local Action Group "The Grabia River Valley" with its registered office in Łask and the public display of video materials from the above-mentioned conference, and I consent to the recording and free multiple dissemination of my image through online and offline transmission of the conference "Debating the future of Europe: contributing to a roadmap for a more united Union "by the Local Action Group "The Grabia River Valley" with its registered office in Łask

INFORMATION CLAUSE

Pursuant to Art. 13 sec. 1 and sec. 2 of the general Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC, I inform you that when processing data personnel in the Local Action Group "The Grabia River Valley":

1. The administrator of your personal data is the Local Action Group "The Graba River Valley" with headquarters in Łask, ul. Słowackiego 14, office 8, 98-100 Łask.

Your personal data will be processed on the basis of art. 6 sec. 1 lit. a GDPR for online and offline transmission of the conference via the Internet by the Local Action Group "The Grabia River Valley" and the publication of video material from the above. conference by the Local Action Group "The Grabia River Valley"  on the Internet.

4. Your personal data will be processed throughout the duration of the activities for promotional or statutory purposes of the Local Action Group "The Grabia River Valley", and after the completion of the activities, they will be stored for the purpose of compulsory archiving of documentation for the time specified in separate regulations.

5. You have the right to access your personal data and the right to rectify, delete, limit processing, the right to object to processing (the right to transfer data.

6. You have the right to withdraw your consent at any time without affecting compliance with the right of processing.

7. You have the right to lodge a complaint with the supervisory authority when you feel that the processing of your personal data violates the provisions of the general Regulation 2016/679.

8. Providing your personal data by you and consenting to their processing and dissemination of your image is voluntary, and in the event of refusal, it will not be possible to participate in the conference.

9. Your data will not be processed in an automated manner and will not be profiled.
